When processed, your pay-cheque stub for your pay-cheque dated October 30 will bear the name M.A.U.T. LIB under code number 505. This deduction will also be recorded on your T-4 slip.**N.B. Members must withdraw in writing to the Secretary of MAUT Librarians Section by September 1 in any given year in order for annual deductions to cease.
Return the completed form BY SEPTEMBER 20. If the deadline is passed, please send in your completed application form regardless. Your payroll deduction will appear on a one-time basis on a later paycheque for the year in which you submit your application and, in subequent years, on your paycheque for the period October 15-30.

Your annual $20.00 membership fee for the MAUT LIBRARIANS' SECTION will appear as an automatic deduction on your paycheque for the period October 15-31 this year and in subsequent years.
To withdraw your membership and the annual membership fee deduction, you must declare your intention to withdraw IN WRITING, to the Secretary of MAUT Librarians' Section, by September 1 in any given year.
